Where be slam?

Several years ago, I recall making and using a SLAM hotkey as a barbarian shammyknight. Now I don't see the skill listed anywhere, much less making a hotkey of it. What gives?
The best solution is not either/or, but best-of.

Action Window >> Combat tab >> "right click" a button for pop-up >> choose "slam".
