Technically, The Nexus connects to all of those plus North Karana, Greater Faydark, Great Divide, Dreadlands, and Toxxulia Forest. It looks like you could simplify the connection lines by flipping the locations on your chart of The Nexus and Shadowhaven.

I believe Kaesora is an indoor zone.
PoJustice is an outdoor zone.
I believe Torment is indoors.
Kedge Keep, I think is indoors too.
Vex Thal is outdoors. (Remember the posts about horses causing trains? ;p)
Grieg's may be outdoors.
Permafrost is indoors.
Katta might be outdoors, I can't recall.
I don't know about Fungus Grove being indoors...

Zone connections:
NPC in tranq will take you to PoHate for a stone. (1 stone per person per port)
VP zone outs all lead to skyfire now.
It's a one-way trip from ToV to PoM and one-way from GD to PoM. PoM zone out takes you to Cobalt Scar.
What's with the one-way from Nexus to Shadowhaven?
Icewell can take you to the Great Divide.

Naming:
Ahsengate should be spelled Ashengate. There are raid instances of Vergalid, Ashengate, Valdeholm and Frostcrypt.
The raid instance of Dreadspire is called Demiplane. It is the same zone though.


PoR:
You left out Skylance, an indoor instance (both group/raid) off of Arcstone, that connects one-way to Relic.
Razorthorn (both group/raid) is an indoor instance off of Sverag.
Deathknell (raid only) is an indoor instance off of ToB.


DoD:
There's a shroom instance off of Corathus, also a mines instance off of that zone.
There's a wurine instance off of Undershore.
There's a witheran instance off of Stoneroot. Also Hive instances.
Illsalin has a couple instances off of it.
The Hive has a couple instances off of it.


Velious:
Kerafyrm's Lair (indoor zone) is off of Eastern Wastes.
Iceclad has the wrong color icon for guild hall portal.


PoPower:
It's a one-way ride from Tranq to PoTime.
In PoTime tier 4 you can go one-way to PoTactics, PoTorment and PoNb(? never took this one lol~)
In PoTime tier 5 you can go one-way to PoFear, PoHate, PoTactics and Crypt of Decay.

Yeah, fixed PoK stuff, just haven't uploaded the new one yet.

For instances, I'm only doing the 'major' ones, that are full zones. I don't consider most or OoW or DoD or PoR mini-instances to be major zones; I do consider CoA and DoB and ToB to be major zones, though, due to a combination of size and importance.

The mini-instances they unhelpfully clutter the map.

One thing that I would look into changing is how you have your Planes set up. IIRC you have the Inner Planes which are the elemental planes and then you have the outer planes which is most of the others. If you organized all the planes together and kep their links there, it would be more logical. This does screw up PoP and TSS though because you have situations like HoHb being and inner zone of HoHa which is actually part of the Plane of Valor. Or BoT being part of Storms. Or Dev/Sevrag/Razorthorn all being part of the Plane of Rage. Or Theatre of Blood is actually the Plane of Music.

As to Instances, I think any instance that does not have raid content should be excluded.

I'm pretty sure that Katta Castellum is an outdoor zone. You can't levitate there, though.

The Twilight Sea laps up against the shoreline on the map at http://www.eqatlas.com/luclin/kattacastellummap.html and I seem to recall running around with bard speed when I took my bard alts there to get their KC songs and turning bard speed off to make it safely down the cliff-side path.

I should take my newest bard alt there soon to confirm which it is and get her KC songs.
